• Click on the Sign Up Now option provided in the middle of the page.

• Enter the activation code as provided in the enrollment letter in the first field.

• Provide the last four digits of the social security number in the second field.

• Enter the date of birth in the final field in mm/dd/yyyy format and click on next.

• In the next pages follow the steps to create a username and password of the portal.

• If you do not have any activation code click on the Sign Up Online button.

• Enter your first, middle and last name in the fields provided.

• Enter the complete address in the provided fields.

• Provide other personal information like date of birth and gender.

• Enter the last four digits of the Social Security Number in the field provided.

• Provide the email address and verify it.

• Tick the box and complete the captcha and click on next.

• In the next steps your details will be verified and an activation code will be sent to the email regarding the activation of the MyChart account.

• On the next page click on the Patient Portal Login option.

• On the landing page enter the Username and Password and click on Sign In.

• In the landing page choose a specialty that you want to check.

• Choose the type of appointment in the next step.

• Enter the zipcode of your locality.

• Choose the time and doctor you prefer to visit.

• Confirm and schedule the appointment and write the reason for the visit and click on schedule it!.
